CONTEST RULES: The individual that sells the most tickets will throw the first pitch at July 17th Common Ground Foundation Day with the Chicago White Sox.
1. Download the order form below and follow order instructions carefully.

Make as many copies as you need, there is not a limit of tickets allowed to be sold.
2. Complete order forms with ticket purchaser name, contact info and collect payment in full via check or money order
3. Tally the complete number of tickets sold and email the following information to info@commongroundfoundation.org by Friday June 19th
a. Contestant name submitted on cover sheet
b. Complete number of tickets sold
c. Date order is mailed or faxed to White Sox Sales Dept
d. Contestant name, phone number and email
4. Submit all of the orders at one time via MAIL OR FAX (Please find address and fax number on order form -orders will not be taken at US Cellular Field ticket windows)
INCLUDE THE FOLLOWING:
a. Cover sheet with contestant name
b. Completed order forms
c. Payment in full
ALL ORDERS MUST BE RECEIVED BY Friday June 19th
5. Winner will be announced via the Common Ground Foundation website and personally contacted on July 1st.
